Immunization Requirements for Students
 
You can verify DBHS has received your proof of vaccination in 
Parent Portal.
Go to the MEDICAL drop down menu.
Click on IMMUNIZATIONS.
Look at comment field in the DTP row.
If you have met the requirement, you will see "Tdap met".
 
 
For the 2011-2012 school year only, all students entering the 7th, 8th, 9th, 10th, 11th, and 12th grades will need proof of a Tdap booster shot before starting school.
 
This requirement:
  • Begins July 1, 2011
  • Will be met by the California Department of Public Health (CDPH) recommendation to receive one dose of Tdap vaccine on or after the 10th birthday.
  • If a student received a dose of Tdap on or after the 7th birthday the student will meet the school immunization requirement.   
  • Applies to all public and private schools.
  • Does not affect students enrolled in summer school.
 
Prior to entering school in August of 2011, proof of a Tdap booster will need to be brought into the health office.  Please do not wait until the last minute to do so.  Print and take this message with you to your medical care provider or local health department (see below) to get the shot ASAP.  NO SHOTS/NO SCHOOL Fall of 2011.  Vaccines that meet the Tdap requirement are:  Tdap, Adacel, Boostrix, DTaP or DTP.  
 
A parent requesting a Personal Beliefs Exemption for the new pertussis requirement MUST sign the new Tdap Personal Beliefs Exemption Form, even if there is a previous signed waiver, no exceptions. The previous waiver will not meet the requirement.
